REPORTING & REPORT WRITING

The first rule of writing an incident report is to investigate thoroughly by asking "THE RIGHT" questions of everyone involved and anyone who might have witnessed anything and then to accurately state the facts without commentary, embellishment or speculation.

An incident report might never have life outside of a file folder, but it should be prepared professionally and completely nevertheless, because if there is lawsuit relating to incident, the report will be a central piece of evidence subject to close scrutiny by judge or jury.
